MDRT Conversion Factor Document

Reference this document to see what revenue counts toward the Production Criteria. Production must be expressed in USD using the MDRT Conversion Factor for the given year.

Membership criteria form

This template should be completed with the applicable names and MDRT IDs for your agents who were 2023 MDRT or MDRT Academy members.

Company verification form

This form is only included here as an example and is not to be used in your award application. During step 8 of the award application process, the applicant will be instructed to download a prepopulated company verification form. The applicant will send that form to their company official for review and signature. The applicant will then upload that signed form to their award application.
